The following list contains our “House Rules.” We ask that you take a minute to
review them as they will be strictly enforced.

First time violations will recieve a 30 day suspension. Second time violations will result
in termination of your privileges.

1. Turn off cell phones.

2. No children, pets or guests.

3. Only one visit per day. You MUST present your original
Doctor Recommendation and CA I.D. at the door.

4. No smoking around the premises.

5. Only purchase for yourself; you cannot buy for others.

REMEMBER IT IS YOUR RESPONSILIBITY TO CARRY YOUR DOCTORS RECOMMENDATION WITH YOU AT ALL TIMES
